The problem is that ITEM_TREE has a root node which is a cluster - this is
fine from a programming point of view, but has two consequences:
1. It is locatable and as such forms part of the path - meaning that
ITEM_TREE has two path steps before we get any data whereas the other
ITEM_STRUCTURE subtypes have only one
2. People might put clinical information in this cluster when in archetype
terms none is justified (since from the archetype point of view, clinical
modellers will think of the ITEM_TREE object as the "root" object of the
structure.
