Allow mixture of W, D etc in ISO8601 Duration (deviation from standard)

Currently, openEHR follows the ISO8601 standard strictly. For durations this means that
PnnW or PnnYnnMnnDTnnHnnMnnS are allowed, but not a mixture. However, in clinical medicine
it is common to specify durations of pregnancies in a combination of weeks and days.

Change Description

Changes: - Relax the definition of syntax allowed in the assumed ISO8601 types in the Support IM so that nnW can be mixed in with the other duration designators, e.g. to form P39W3D. - Relax the duration constraint specification in both ADL and ADL2 specifications so that PWD can be used in archetypes.
