Unable to carry rubric in TERM_MAPPING

Description

Since mappings are CODE_PHRASE there is no way to carry the rubric of a mapped code.

It would also be good to carry an uncoded mapping where we want the value to be coded but mapped back to a free text value.

Suggest updating Mapping to include a DV_TEXT or DV_CODED_TEXT

Adding a optional value attribute to TERM_MAPPING would be one option although backward compatibility is affect the purity of the model.
Alternatively, the proposal is likely to be sufficient because I can't think of a reason to carry an synonym/original text and a preferred term in a TERM_MAPPING

I'm assuming also that suggestion would be good enough, and won't break anything in XSD land.

There are a several use cases, for example, people from the Norwegian project use snomed to code text in norwegian, but as snomed is yet to be translated into Norwegian, the original English term should be added to know the term it was referring to.

Also, for validation purposes is better to have the term alongside the original text, in order to allow humans to verify assigned codes
