Add an attribute to ELEMENT to record 'null reason'

Description

In various situations identified in the past, a reason needs to be included when there is null data, e.g. if patient refused to provide information. It is therefore suggested to add an attribute such as null_reason: DV_TEXT to ELEMENT.

What's the progress on this request? It's come up again today with a practical use case.

I think we should add this extra attribute to let users, when needed, give a specific reason. It makes sense to provide some extra information if you break a hard constraint like mandatory .
