HiGHmed-seminarium

14:00-15:30

@Erik Sundvall, @Erik Sundvall (@gmail profile) & @Erik Sundvall (@sundvall.org)

HiGHmed presenterade sitt arbete för mötet utifrån publikens frågor.

Information inför seminariet:

HiGHmed is a German medical informatics initiative which is working on novel, interoperable solutions in medical informatics with the aim to make medical patient data accessible for clinical research and education which will, in turn, improve patient care, Home . To do that HiGHmed develops a joint cross-institutional reference architecture based on relevant existing standards like openEHR and FHIR. HiGHmed therefore has extensive knowledge in using openEHR.

During this seminar Birger Haarbrandt  and Antje Wulff from HiGHmed will discuss and share their experiences on using openEHR. The seminar will not include any regular presentation. Instead the audience is expected to see at least on of the recorded presentations below before the seminar starts.

Basic orientation:

Some questions (& potential scaffold for notes) at the HiGHmed seminar

  • How does the German archetype translation process work? Is it described somewhere?

  • To what extent do you use SNOMED CT?

  • We have understood that you in addition to openEHR also are using HL7 FHIR. What do you use HL7 FHIR for and how do you handle the intersection between openEHR and HL7 FHIR?

  • Management of HiGHmed-produced data models: How do you keep track of all (future) templates? E.g. detecting/defining potential reusable of sub-templates and terminology lists/selections/subsets/refsets? How do you keep track of reusable AQL queries?

  • HiGHmed is one of four german “sibling” informatics initiatives. Is it likely that the openEHR parts will continue to be central in the next financing/project-phase? If they will not become core in national efforts will the current participants continue using openEHR internally and among each other?

  • Region Östergötland (and likely soon SFMI) are experimenting with openEHR for Echocardiography and have been looking at the corresponding HiGHmed template at https://ckm.highmed.org/ckm/templates/1246.169.85. Is it actively used for data collection now? Would you be interested in collaborating regarding making an updated more international and SNOMED CT coded template?

    • Could it later be used for multi center studies with Swedish and German sites?

    • Do you think it would be beneficial making specific archetypes for echocardiography or common groups of heart measurements (possibly supporting multiple modalities like (ultrasound and MR)?

    • Do you use LOINC in this or other contexts?

  • How are the templates in the HiGHmed Oncology project at https://ckm.highmed.org/ckm/projects/867.116.5 used? Is it for primary data capture or mostly for reporting/statistics?

    • Background: There is an upcoming Swedish project regarding openEHR+SNOMED CT based pathology reports (primary data capture macroscopic+microscopic), likely starting with breast cancer. Region Stockholm is also looking at openEHR based surgery notes. In both cases the identified Swedish data capturing needs seem to in part be a bit more detailed then the templates found in the HiGHmed Oncology project at https://ckm.highmed.org/ckm/projects/867.116.5

  • What are you up to regarding Genomics with openEHR in HiGHmed?

  • Do you have questions to us? Are you looking for any specific collaborations/discussions?

  • We’ve heard of interest in the HiGHmed approach from the Netherlands, can you tell us a bit more?

  • How do you do the archetype version management? Any changes or updates to an archetype could cause problems with existing data. What have you done to overcome this?
