Update Entry package design principles in EHR IM

Description

Currently, section 8.1.1 is slightly out of date. A pre-publication paper by Heard and Beale is cited, when there is now the MedInfo 2007 paper available. In addition, the labelling on the MedInfo 2007 version of the ontology (i.e. typology) is clearer than that shown in the EHR IM.
