Missing 204 responses in some endpoints

In some POST/PUT endpoints the 204 response is not included in the "Responses" section. In POST /ehr and PUT /ehr the 204 is missing, in PUT /ehr_status the 204 response is there. In POST/PUT /composition there is no 204. For POST /ehr/id/directory there is no 204, for PUT there is. We agreed in the REST WG to add the missing 204 response to the doc.
