I think unless a clear problem / need exists here, we should reject or obsolete this PR.

I'm not sure what kind of feature this will add and/or how this would work in practice.
Is there some references to documentation from other standards like the mentioned ISO/FHIR?

I am not overly in favour of this proposal but I will say that it is in the ISO healthcare identification standard and FHIR still has this concept and realm is more general than country, it could be less than or greater than a country. It depends on the identifier.

The current DV_IDENTIFIER datatype lacks a Realm attribute, which is required for compatibility with a number of international standards.
